Dear Referee/Assistant Referee,
Thank you for taking the time to assist our league in producing the most educated and professional coaches/referees possible. We appreciate both your positive feedback as well as items to improve upon. Please use the form below to help evaluate coaches/referees. Please fill out this form as clearly as possible, using very specific details. Evaluations will remain anonymous to the coaches/referees unless otherwise requested by the person filling out the form. Remember all our coaches are unpaid volunteers and the majority of the referees are youths. It takes a great amount of time, selflessness, patience, and knowledge of the game, among other important attributes, to step up to these important jobs. These evaluations will be used to give feedback and constructive criticism to coaches/referees to help them to learn and better themselves. Overly nasty or otherwise derogatory comments will not be tolerated. Again, thank you so much for your time. Your assistance is greatly appreciated and will serve to grow and better our league.
